Story of Patient ID Errors

Hello fellow POC Leadership,

Do any of you have any Patient Safety stories you could share to show how easy it is to make patient ID errors in POC? Hoping for something that will demonstrate to my passionate non-lab colleagues whythey must ID/document their downtime testing before we can upload it. We often explain that we cannot provide them information to work backwards or make educated guesses as this increases the chance for a patient ID error, but they sometimes struggle to see the logic in that.
