Critical Result Read Back

I am trying to push forward with a process for PCT/CMA/etc. to document the first and last name of the nurse/physician they report to the critical glucose results to on the Accu-check Inform II device itself in real time. Nursing and education are ready to push this out. My hold up is how to satisfy the read back results COM.30100. How does your facility satisfy this?

We have a canned comment that says "Primary Provider Notified" so as to not have to spell out the name of the provider.  The operator is traceable through our middleware This comment uploads to the chart as part of the result and in training we stress this is the documentation only, it does not actually notify the provider.  I've been through several rounds with TJC and no one has ever argued that this doesn't satisfy the documentation portion of critical results.  

We are also trying to figure out a workable way to document this.  We have Nova and currently the users add a comment that says "RN/MD notified".  However, one of our sister hospitals did get cited for CAP that this was not enough when PCA's/PCT's perform glucose.  Since they are not a treating provider, the CAP inspector stated that the first and last name of the RN or MD notified must be documented.

We have the NA/PCTs comment "critical given to nurse" and then free text and scan the nurse badge accepting critical which records the employee ID. We use the Nova meters so not sure if other meters allow you to do this. We haven't had good compliance with this despite the training and almost of year of audits and leadership rounding with staff. Not sure how to satisfy the read back but I imagine you could have a canned comment "critical given to nurse and read back" and then free text first and last name.
