Fentanyl testing

Is anyone doing POC Fentanyl testing?  I have had several requests for this by physicians in OB clinics.  We do not offer it in our main labs and send out is taking a long time to get results back.  Just curious if anyone else is performing this test.
Thanks
Kim

Hi Kim,
I've been getting this question a lot, too, especially from the OB docs.  Our vendor brought a product from All Test. It's a CLIA Waived Fentanyl Urine Test Cassette.  I have not had a chance to look into it just yet, but it's a very simple test. Hope this helps!

I have seen this test online, but didn't know if anyone was using it. I am curious about the external control.  Which distributor carried it?  I know that it has not been on the market very long.

My rep from Fischer Scientific dropped off a box for me to test out. Although it states nothing in the IFU sent with the box about any type of control, I would definitely want an external control. I just haven't had a chance to look into it, yet.

Hello all,
We are evaluating the Alltest CLIA waived urine fentanyl test for use at our detention center and other locations. We had trouble finding a liquid quality control as the test’s methodology tests for Fentanyl in urine and is not very sensitive in detecting Norfentanyl, (a metabolite) in urine. Eventually we have found these:

  1. Detectabuse® Fentanyl 5ng/mL Positive, 5ml Vial #18008613
  2. Detectabuse® Stat‐Skreen‐H® negative control Vial #19700000

We have not implemented the test yet. We are evaluating the test against our labs test, (which looks for Norfentanyl). For those looking to start this kind of testing it would be prudent to figure out what providers are interested in finding when performing a urine fentanyl test and make sure the test kit selected matches the intent. Fentanyl or the metabolite Norfentanyl.

Note: I know more about fentanyl and the human bodies metabolizing of fentanyl now than I ever intended to know. I'm sure others will end up the same way. Here are a couple of journal articles which helped me understand the differences.

  1. Fentanyl Absorption, Distribution, Metabolism, and Excretion: Narrative Review and Clinical Significance Related to Illicitly Manufactured Fentanyl. H. Elizabeth Bird et al
  2. An Analysis of the Duration of Fentanyl and Its Metabolites in Urine and Saliva. Jeffrey H. Silverstein, MD et al

Hello all,
What was your rationale for choosing the Alltest Fentanyl Test to evaluate in regards to the other waived fentanyl test kits? Has anyone evaluated any of the other fentanyl waived test kits?
