QR Codes for information

We will now be covering 5 hospital campuses and a growing number of physicians' offices. Looking for a way to get communication from all locations to me without a fax machine. Does anyone have a QR code that brings up a document that can have information added to it and then sent directly to you?

This is an interesting idea! Do you have an intranet like SharePoint or a corporate Google docs account? If so, you could create a page with the form you want filled out and then use Microsoft Edge to create a QR code - just right-click on the page and select 'Create QR Code for this page'. There are also websites that can create the QR code for you once you have the form setup.
I have a SharePoint site through my organization for just my POC physician office locations. I have a "POCT lead" in each of those clinics that I grant access to the SharePoint site. They can then upload their documents and save it to the SharePoint, edit documents I create on the SharePoint. Or if they aren't feeling very tech savy, I have them just email me PDF copies of their logsheets and I save them on the SharePoint site for them. This has worked well for us since we started it about 2 years ago. I also save documents ont he SharePoint site for these lead staff to access, such as their POC downtime forms, POC patient logsheets etc.
We link MediaLab documents to a SharePoint site, this way the most current document is available. Are you using MediaLab for document control? If so, you could allow linking from outside Medialab which gives you a web address that could be used to generate a QR code.
