Provider Performed Microscopy Quality Control

Good morning, hive mind.  I have a few clinics that have PPMP CLIA licenses, and I need commercially available quality control materials for the following:

  • Direct wet mount for the presence or absence of bacteria, fungi, parasites, and human cellular elements

  • KOH preparations

  • Urine sediment examinations

Lori,  I would make a policy stating that QC material is not readily available and have staff refer to reference material images (can be something you make, or use the CLSI POCT10-A2 book)

Ensure that they're participating in proficiency testing. This can help serve as competency assessment. We have our APPs check off that they viewed reference images prior to performing a wet mount. These are always available at the bench. We created a booklet from previous years of proficiency test images from API, CAP and WSLH. We also purchased slides with dried Candida but they're very expensive. 

Great suggestions.  Where do your APPs document that they viewed reference images prior to performing the wet mount?

We use CAP images for competency assessment of prescribers.  They view the images and take a test.   The answer sheets and images are kept in the CAP PT binder. 

We were in a similar situation with fern testing when we were inspected by a CLIA inspector.  She recommended if we could not find suitable QC material, we could actually have the providers read the results twice and add that to a patient log where the duplicate results would be recorded. 

We use a paper log where the APPs check off the column for QC. The APP must be the one who orders, performs and results the PPMP. There are columns for everything and the final column on the log is for the MA to verify the clerical accuracy of what was entered into the EMR.
