Anyone have a good training quiz for the Hemochron 100 ACT testing device they could share?

Here is a copy of the competency assessment I made for our GEM100 Hemochrons with quiz included. I made this to very specifically address all CLIA assessment methods with exact wording out of the CFR; our most recent TJC inspection did not turn up any issues with this format.... except not using a patient sticker for CLIA method 2 (section 4 of my form). Our inspector suggested just placing an account number, which would still be traceable while not causing any HIPAA issues.
