Hospital-based EMS

My facility is in the process of starting its own EMS department, mostly for intra-system transports and discharges to SNF or residences. They want to have glucometers for the ambulances and I want to make sure we have everything done appropriately. Those who have similar setups at your facilities, does the EMS "department" have its own CLIA or does it fall under ER's? Does anyone have any suggestions or advice to help the POC aspect of this get started smoothly?

We have this and we got them their own waived CLIA.  Best decision ever.

Ours are under our CLIA.  I would check with your state regs to make sure, but it works for us.  They have Istats and Nova Statstrip, plus pH testing.  The have a system set up now so since they know the patient's information before they go pick them up they pre-register the patient.  They print out barcodes to take with them so they can scan when they do testing.  The have multiple bags set up so they keep their glucometer QC and non-registered barcodes in each one just in case it's needed.  They keep their cartridges in the fridge and take a couple with them on their run and once they are at room temp, they date them and put them on the counter for the team to grab when they get the meters.  They also use smaller insulated bags to put the instruments so it regulates the temperature better.  The Istat can get kinda finicky if it gets too cold.
We have them in QML and monitor their competencies just like any other unit.
  • Make sure they have a process to secure the docking/charging stations in the ambulance. 
  • A process for scheduling QC so they don't get a lockout while they are on a run.  
  • Extra batteries for their meters.
  • A process for reconciling patient information if it's being downloaded to your data manager.
  • a way to combat temperature fluctuations.
