I am trying to create a committee in our hospital system for reviewing new POC test requests, and would like to know for systems that have a similar committee, who by title are the members on your committee? Thanks for your responses!
We don't have a committee, per say. Leadership determines if the test is needed, if POC can meet patient needs better than main lab testing (prevent unnecessary duplication of testing methods), and if cost effective. Leadership is composed of our laboratory director, nursing directors, and with some testing, VPs. POC coordinators have input regarding CLIA requirements, instrument requirements, nursing education, etc.
Our committee consists of someone from our POC department (usually the POC Coordinator who would be overseeing the testing so that fluctuates), POC Lab Manager, our section medical director (pathologist), and our Lab Director. We have a form that locations requesting testing have to fill out and turn in for our approval or denial. I have attached our form here.STL LABPOC Testing Request Form.docx

Mindy...I like your form. I'm going to send it to Kim and see if she thinks this would be something we should be using. I'm not sure if leadership has their own forms/process.
