Creatinine Fail AQH-A 2025

I am inquiring if anyone has received their results from the AQH-A 2025 quality cross check.

A colleague mentioned that their creatinine results failed when they received their feedback from CAP and other sites, specifically regarding creatinine, although CAP did not address this issue. 

Personally, I did not encounter any problems.

My colleague assured me that they followed all procedures correctly, including sample preparation and QA mode, yet they still experienced a failure.

Given that creatinine is a stable analyte, I am uncertain as to why this discrepancy occurred.

Could anyone shed light on what might cause this creatinine discrepancy?

Additionally, did anyone else experience issues with their quality cross check, particularly concerning creatinine?

Hello!

Yes! This happened to me as well but on the API 2025 Chemistry Core 1st event. All of my creatinine values failed on the high end by 0.1 or one value of 0.2 outside the expected range. When I asked API about it, they said CMS narrowed the acceptable ranges for creat, glucose and a few other analytes starting with this event and said they think this is just a result due to the tighter ranges. I've never had an issue with creat at this location before either. 

This is for the EPOC system by Siemens.

Jon, you might want to mention what specific system the testing is being performed on as well that is having the issue.  

Hey Jeremy, you are correct. I am speaking of the epoc but this may apply to other analyzers that measures this analyte

We had no issues on the Epoc with AQH-A 2025 for Creatinine, but we also run the AQHQ (Crosscheck) as an alternative (long story, and it is per CAP recommendation), and we did get an unsatisfactory result for Creatinine for the Epoc.  I'm also noticing that my QC for Creatinine is having a negative bias - anyone else noticing this?  What are you doing about it since we can't adjust means or recalibrate the devices?
