HI all,
Looking to redo our competency forms for i-stat. Are you all including spaces to write the result of their competency and/or including spaces for completion dates of their quizzes/qc/ maintenance checks or are you all utilizing just a checklist with operator/assessor signatures?

I have atached the form I use.
We use a combination of check off, values, and dates. I have attached our competency assessment.
If you are a Joint Commission accredited organization check HR.01.06.01 and you get to do how you want it done provided you meet all the 6 methods of validation. Your policy should state how it is done and you will be surveyed base on your policy.
