Epoc creatinine PT failures

Good morning, 
I've had two API proficiency test failures this year and now need to complete an action plan to submit to JC. 

The first event 4 out of 5 samples failed by 0.1 to 0.2 on the upper limit. On my last survey, one sample failed by 0.1 on the upper limit and 0.5 for another sample on the upper limit. I watched like a hawk the past survey, all analysis and operator technique was perfect. I know the ranges decreased from 15% to 10% this year. This is really having an impact on our facility. 

Is anyone else having trouble with EPOC creatinine proficiency testing?

Did you try to switch Proficiency provider? Have you stop doing the test in the interim as 2 failures calls for “Stop testing” while you completing the action plan.

You can buy an off cycle proficiency sample from another provider, run it and self grade and that will be part of your action plan.
