Education documentation requirements for moderate complexity testing

Hello All,
I hope to better understand the education documentation requirements for moderate complexity testing personnel. Specifically, do you require primary source verification for diplomas and transcripts? I have referred to 42CFR 493.1423(b)(1-4) and understand what is required of the testing individual in order to meet the requirements. However, in a recent discussion the possible need to use primary source verification for moderate testing personnel documents was raised as a concern so I thought I'd reach out to the community and see what others are doing. I appreciate the help!

In regard to your inquiry relating to POC.06920 -Competency Assessment- Assessor qualifications:
Answer: A verification of the degree or diploma that lists the degree obtained is necessary to have on file to meet the intent of the requirement. Please see the area highlighted in yellow below for where this information can be found.
Per GEN.54400 A state laboratory personnel license specific to the role and specialty of testing may be used instead of a diploma, transcripts, or a PSV report if the laboratory is located in a state that requires laboratory personnel licensure and qualifications are at least as stringent as CLIA (licensure for other disciplines, such as nursing, radiology, or respiratory therapy are not acceptable).
