CRRT Blood--iSTAT

Hi all, 

Just reaching out to see if anyone performs ionized calcium testing on CRRT (continuous renal replacement therapy) blood on the iSTAT. One of our physicians from Peds Nephrology has been very interested in it, but we were hoping to gain some insight on how other facilities are classifying this sample type on the iSTAT and how the results are distinguished within the EMR. We will be transitioning to Epic this Fall.

We do perform CRRT testing for both the iCa and ACT on the ISTAT. In Cerner it was built to show in the EMR as POC Calcium Ionized CRRT and POC ACT CRRT. In DE, we added a source of CRRT to the unit that performs this test. They have to option to select in on the ISTAT screen. We use Telcor as our middleware. We too will be transitioning to Epic but not until next fall. I do not know how this will impact things here.

  We are a large health system and at our pediatric facility, we have much the same process as Debra described.  Here is how it currently appears in Cerner - clearly identified as a CRRT sample.

  We are also transitioning to Epic later this year and have yet to work out an analogous process with Beaker/Epic, but we are working on it!

You might want to check with CAP about this type of sample as I was told it was not one that Abbott submitted as a sample source for FDA approval which if you do the testing on the ISTAT makes it High-complexity.
