Carboxy- and Methemoglobin testing Advice

Hello All,
I wanted to get advice and info on co-ox testing.
We currently use the AVOXimeter 4000 for carboxy and methemoglobin testing. It is located in the NICU.
In addition to the NICU patients, we also have ER patients needing testing, but this usually only during the winter months.
Our SICU is located on the other side of the hospital, and they have recently started ordering Co-ox testing.
  1. Our Nicu respiratory therapists run the tests.
  2. We have 330 bed hospital, 2nd busiest ER in Louisiana, about a 40-bed ER, 14 bed SICU, and 8 bed NICU.
  3. Our volume of co-ox testing averages to about 10 tests a month-not including O2 sats in the cath lab.
  4. We use epocs for blood gas and poc testing, and the istat for ACT testing.

1. is there a handheld co-ox testing device?

2. I have looked for literature on the stability of blood samples for carboxy and methemoglobin testing, and i found the most conservative estimate to be within 30 minutes. Do y'all have any more info?

2. How do you have your co-ox workflow set up?
  1.    Does the main lab handle it?
  2.    Do you have more than 1 table top analyzer?
  3. Do the samples get tubed between departments?
  4. Is turnaround time an issue if the samples are sent to the lab or tubed/walked to another department?
Thanks for any and all Info!

Jolie,
1. No hand held co-ox that I am aware of. The AVOX 4000 has been discontinued and the cuvettes in inventory are all they will have. I think they are supposed to run out in late 2027 by Werfen projection.
2. I (when I was working-retired now) had 14  or so AVOX 4000 over 7 hospitals. Any co-ox was sent to the lab in a green topped tube either from NICU or ER through the tube system. All Cath Lab for just OxyHGB samples were done in the Cath Labs by nursing personnel. Lab personnel were told to run any co-ox immediately.
3. For the stability have you looked in the AVOX manual? I think 30 minutes is what I remember. We only did them on babies on Nitrous Oxide and only two NICU used NO. COOX Volume over 7 hospitals was
about 250 a year counting house fires for CO and NICU for MethHgb.

Let me know if you have any other questions.

We use AVOX1000 in our CCL for Thgb and OxyHgb. We have 12 GEMs around the hospital for Carboxy and Methgb, including in the NICU.  If the NICU GEM is warming up a cartridge or something that will casue a delay, then our RTs can run up 1 floor to the PICU.   It is stationary, but you can put GEMs on a cart. Just be careful when wheeling it around and remembering to plug it in so you don't lose the cartridge. If a patient needs that testing that is not in one of the 12 areas, that is sent to Main Lab.  Main Lab has 2 Radiometers. Gases are sent through the tube station and supposed to be immediately walked back to the Chemistry dept.
