Form for Glucose Vendor Fair

Has anyone recently held a Glucose Meter Fair for end users? I am looking for a few points for staff to assess the various meters when the visit the 3 vendors at our Fair. A sample form that you have used would be a bonus!

We have a system wide Skills fair every fall and it incorporates all the New product info, recertification on meters/POCT, Blood bank banding, ergonomics, etc and it involves all nursing staff, respiratory therapy, POCT, OT/PT, nurse educator and administration. They get a packet that has to be signed by a proctor at each station as proof of attendence to each booth needed to recertify them on that product or information.
I ask them to go online and complete my quiz in RALS-web before the skills fair for electronic audit trail and the proctor signs off that they ran the meter and performed QC (also visible electronically) and I have them perform a proficiency sample as a mock patient.
The whole fair takes about two hours from 1st to last booth and its a whole lot less chasing people constantly to recertify on their anniversary month.
sorry I need to clarify I am looking at a form for staff to evaluate the 3 vendors (roche, abbott and nova) at a glucose vendor fair.
anyone have this information to share?
