Eye pH - POC test or not

The eye pH discussion is going on at our facility.  Is it a POC test or not since no "specimen" is being collected?  Are you using the strips in tape form or the long ones with multi-pads and if the long ones, how does that work for an eye?

It shouldn’t be considered a Lab test since no specimen is collected.

We treat this as a POC test.  While a "specimen" that can be labeled is not collected a specimen is collected on the pH paper.  To me the specimen is very similar to the specimen collected for whole blood glucose, in that it is not labeled and then is discarded with the test strip.  We use pH test paper on a roll and the doc can tear off the amount desired to perform the test. 

We do the same as Rick. I had the same analogy too but he beat me to it. lol.

It's a bit of a grey area and I've seen it defended both ways.


We cover ourselves by defining it as a POC test and use the Hydrion plastic pH indicator strips from MicroEssential - range is 0-14.  These easily lend themselves to that setting and also offer a clear expiration date on the canister.  Some of the older formats of pH paper left that up in the air.

We were cited for not having QC and Competency performed during a past inspection (NYSDOH). Since then we have had to treat it as a POCT test.


 


It is a POC test  because you use lacrimal fluid to test. 


Just like amniotic fluid nitrazine pH, you do not collect sample in a vial but you use the fluid to test.


It is a lab test according to JCAHO and State of Tennessee surveyors. 


I like this analogy.  You’ve changed my mind on it LOL.

That is the great thing about a listserve...lots of experience and knowledge from other POCC's makes all of us smarter and our programs better!


This is a copy of an email we received from CAP concerning eye pH during one of our CAP inspections.  We have not considered this a POC test since.  We also
got a written report from our ED group stating it was a procedure, not a test.

It is a procedure not a test. We do not charge for it as a separate  POC test.


It does not get reported under  Lab POC test in the EMR.


It is recorded in the Nurses Notes.


We do the same with Amniotest PH swabs. Not ordered,


not charged,  not reported under Lab POC, only at Nurses Notes.


It is a procedure not POC test.  


We put it under POC so the QC will be done on each day of use.


We want to make sure the PH paper in use is giving the correct color reaction.
