Department has no Glucometer but might need CBG testing

Hi All,

Our infusion center has no glucometer, but might need to do a testing on a patient because of the procedure they're undergoing. The  Wound clinic next door, however, has available glucometers. Is it okay to use those meters (transporting the meter instead of the patient) and provided an authorized user will perform the testing? I did suggest if the patient could come to our outpatient department since they can do the testing, but it is outside of our hospital and I am not sure how mobile the patient will be to go back and forth, also the wait can be long. I am aware that the meters are department-specific in our facility, but I'm not sure if that's just because of the location that the meters have to be assigned. We have the Nova StatStrip meters by the way. Looking forward to your kind responses as I am new to navigating this role!

You didn't state what regulatory body you are accredited with, if CAP/JC or other

As you mentioned, provided the user has completed the certification steps to use the glucometer... *maybe*.  We have departments in our campus that share meters and its acceptable.

Other things to consider that are not clear in your question: Are both locations under your laboratory's licensure umbrella, are they both part of your campus?  You mentioned one being outside of the hospital. For example, we have locations that are across the street that fall under the campus umbrella and are OK, but physician offices on campus that are NOT.  We also have to deal with "for-profit vs  non-profit" depts, for-profit depts can not use our glucometers. 

Other:
  •  If your using positive patient ID on the StatStrips, you would want to make sure these patients are being uploaded into the analyzer for this purpose or the glucometer will not recognize them.
    • ADT/patient ID feed.
  • You'd want to add the Infusion Dept to your list cited departments that use the glucometer to your regulatory body.

we use Nova StatStrip, NovaNet middleware, Telcor QML; POC system middleware.

We are CAP/JC accredited. The wound clinic and infusion clinic are inside the hospital which will fall under the hospital lab's CLIA, correct? Our outpatient lab has its own separate CLIA. They do might have an issue with the patient ID, depending on how they're registered. Thank you for the clarification! I will look into the list cited department.

  • *IF*, they are part of your "Hospital" and not split out then, yes.  Being careful here of between the differences of physically being in the same building vs being legally part of the Hospital. 
    • Like I said, we have locations on site that are not part of the "Hospital" they are primary care/physician offices/private practices or "For-Profit" departments that are split out and therefor can not use the hospital glucometers.
    • Our hospital is "non-profit".

When in doubt, call your Accrediting agency and ask.
