Annual or Biennial Verification of IQCP – Process Inquiry

Hi,

I would like to ask how everyone verifies their IQCP on an annual or biennial basis. Is there a specific procedure or process that is followed to ensure the continued effectiveness of the IQCP?


If anyone is willing to share their approach, it would be greatly appreciated.

We review annually, typically we review the previous year every Jan/Feb.  We have a form that includes each QA Process/Monitor that is covered by the IQCP (Patient ID, Collection, Training, reagents, etc) and we describe any issues identified, like failed QC or patient misidentifications, throughout the year and what corrective action was taken.  Then we have a review of the effectiveness of the corrective action, if any changes are needed to the IQCP, etc.  All of it is reviewed and signed off by the Medical/Section Director.

Thank you for your detailed response. I have a few follow-up questions to better understand the process.


First, during the month or week of the IQCP review, do you ever run QC daily instead of following the QC frequency stated in the IQCP? I’ve heard that some hospitals temporarily run daily QC to verify that the instruments are performing as expected.


Second, do you document every QC failure and the corresponding corrective action for the entire year? I imagine that could result in a large amount of documentation.


Third, do you use the same verification form that is available on the CAP website, or do you use a customized version or something different?

We have our own form.  Each QC failure or repeat and corrective action is documented within the middleware and notes are visible on our LJ reports.  Anything significant is mentioned and the corrective action reviewed as part of the IQCP assessment.  We don't increase QC frequency, just establish that the current frequency is acceptable per the IQCP review.
